The NHS introduced Category H in March 2026 as part of a reform to the Drug Tariff.

It replaces elements of Category C for medicines supplied by multiple manufacturers. Although the update appears administrative, it has implications for pharmacy profit margin, purchasing behaviour, and reimbursement stability. 

 

Why Category H was introduced. 

Category C has historically relied on a single reference price, usually tied to one brand or supplier. This caused reimbursement to diverge from the real prices pharmacies paid in the market. Pharmacies often purchase the same product at several different price points depending on supplier agreements. 

The fixed reimbursement rarely reflected this variation.

Category H aims to correct this by using weighted supplier data, which is more consistent with modern approaches to pharmacy business intelligence and pharmacy data analytics. 

 

How the model works. 

NHS BSA and DHSC collect quarterly supplier information under the 2018 disclosure regulations. Reimbursement is calculated using weighted averages that reflect real market activity. This method is closer to the type of monitoring used in retail pharmacy analytics and pharmacy benchmarking. 

For example, using simplified data for isosorbide mononitrate 25mg modified release capsules: 

  • Manufacturer 1 supplied 5,449 packs at £3.80 
  • Manufacturer 2 supplied 3,547 packs at £3.21 
  • Manufacturer 3 supplied 27,946 packs at £3.46 
  • The weighted calculation produces a base of £3.48 

Applying a typical margin and discount deduction results in a reimbursement of £3.96, compared with the previous Category C price of £4.01.

Even small differences like this shift the overall margin position. 

  

Impacts on pharmacy margin. 

Category C has historically offered a helpful margin where acquisition costs were lower than the tariff. Category H reduces the gap between reimbursement and actual market costs.  

This may produce steadier outcomes in some cases, but it may also reduce flexibility in others. Pharmacies may see reimbursement that follows real purchasing behaviour more closely. 

 

Operational considerations. 

Category H uses data from previous months. If market prices change rapidly, reimbursement may lag behind. Pharmacies may experience challenges when prices rise faster than the weighted reimbursement reflects. 

Price concessions may remain necessary when reimbursement falls behind acquisition cost trends. Suppliers may also adjust availability if reimbursement levels reduce commercial viability. 

Strong visibility of purchasing patterns, cost movements and supplier behaviour will support more stable planning. This is an area where pharmacy business intelligence, pharmacy data, and related analytics tools become increasingly important for owners aiming to manage risk. 

 

Longer-term considerations. 

Category H reflects a shift toward reimbursement based on actual market conditions. The reform may reduce the scale of overpayments that affected other tariff categories. However, quarterly updates may not capture sudden changes, so some unpredictability remains. 

Only 11 products are included at the initial stage. Broader effects will take time to emerge, and ongoing monitoring will be needed before firm conclusions can be drawn. 

 

Conclusion. 

Category H acknowledges the complexity of a market with multiple manufacturers. It seeks to create a reimbursement model that aligns more closely with real acquisition costs. This can support fairness but may also narrow margin opportunities and increase the need for careful forecasting. 

Pharmacies that actively track pricing trends and maintain awareness of supplier behaviour will adapt more easily. Many will rely on pharmacy business intelligence, pharmacy analytics software, and structured data to understand the changing environment and protect revenue.
